Insurance Claims for Physical Therapy

Do you have a physical problem, such as low back pain, tendinitis, neuralgia or TMJ syndrome?  If so, you are a good candidate for Physical Therapy and may be able to make a Physical Therapy insurance claim.

If your insurance provider allows you to go out of network (PPO), we will help you get reimbursed for your treatments.

We will give your a typed receipt which you can attached to your carrier's claim form and submit.   Most claims are processed smoothly.

Payment is made at each session by cash or check.  The initial session, which includes initial evaluation is $240.  The follow up treatments are $160. 

Frome Physical Therapy does not accept assignment and is not a part of any HMO, Medicare or Medicaid Plans, and does not make, "No Fault," or "Workmen's Compensation" Claims.

To avoid being charged for your session, please give us 48 hours notice for cancellations. Sessions begin and end on time, so please be prompt.
